What to do in the event of an accident.

On the platform, for all warranties (except the glass warranty with repairs at approved centers):

Complete the FNOL form in its entirety, specifying in the notes:

SELECTED BODY SHOP WITHIN THE QUIXA NETWORK.

  • SELECTED NON-APPROVED BODY SHOP.

Additional documents to be uploaded based on the warranty involved in the accident:

  • TOTAL AND PARTIAL THEFT

Copy of the report filed with the authorities. If the event occurred abroad, the report must be filed with both the foreign and Italian authorities.

  • FIRE AND THIRD-PARTY CLAIM FROM FIRE

Copy of the report filed with the authorities and a full copy of the fire brigade report, if any.

  • NATURAL EVENTS

In the event of an accident involving an uninsured vehicle, a report from the authorities who responded to the accident site or a report to the competent authority.

It is not necessary to file a claim together if you contact an approved repair center.

In the event of glass damage, contact one of our approved repairers, Carglass, Glassdrive, or VetroCar. They will file a claim and perform the necessary repairs at the most advantageous conditions for you.

If, however, you intend to go to a trusted repairer, remembering that deductibles and excesses will apply as per the policy terms, contact the Company through the Intermediary.

The Company may request the customer to submit additional documentation for claim management, as provided in the information package.

MAPFRE GAP Policy

Pursuant to Article 1913 of the Italian Civil Code, in the event of a Claim, the Insured must promptly notify the Insurer in writing no later than 3 days after the Claim occurred or the Insured became aware of it, sending the Claim report and the documentation indicated in the information set (in Article 7 – "Claim Reporting Procedures") via one of the following methods:

Registered mail with return receipt to MAPFRE ASISTENCIA S.A., Strada Trossi 66 – 13871 Verrone (BI).

As indicated in the information pack (Art. 7 – "WHAT TO DO IN THE EVENT OF A CLAIM"),

The Insured must report for all claims:

a) a clear and detailed description of the accident and any information useful for assessing and settling the damage;

b) the Insured's contact information (telephone and/or mobile number, email address, etc.);

c) a vehicle history statement with a record of loss of ownership issued by the Public Automobile Registry (PRA);

d) a copy of the vehicle's certificate of ownership and registration document, or a copy of the single vehicle registration certificate;

e) a copy of the purchase documents and purchase invoice, indicating the vehicle's purchase price and the dealer's name;

f) for leased vehicles, a copy of the leasing contract and the leasing company's authorization to pay the compensation to the Insured;

g) a copy of the current, active, and unsuspended third-party motor vehicle liability insurance policy in effect at the time the insurance coverage commences and in effect, even if suspended, at the time of the accident;

h) a copy of the documentation certifying the purchase of the New Vehicle, including the invoice indicating the name of the selling Dealer;

i) for a leased New Vehicle, a copy of the new leasing agreement and proof of payment of the purchase invoice for the New Vehicle by the leasing company within 180 days of the date of the accident;

Contractual Conditions for the Insured

j) In the case of vehicles that have undergone technical modifications/transformations, a copy of the type approval certificate;

k) Claim settlement receipt relating to the Land Vehicle Hull Policy;

l) If the Insured is VAT registered, a declaration relating to the VAT recovery regime on the purchase of the vehicle.

In addition to these documents, the Insured must provide the Insurer with the following additional documentation depending on the type of claim:

In the event of Total Theft:

m) A copy of the Total Theft report of the vehicle filed with the Public Authority;

n) All keys supplied with the vehicle or certification of their delivery to the insurance company of the policy.

Land Vehicle Hulls or other insurance policy insuring the Vehicle for the same event as the Claim;

In the event of Total Fire or Destruction due to Total Damage to the Vehicle, or discovery of the Wreckage:

o) Copy of the Vehicle's scrapping certificate from the authorized dismantling company or declaration of acceptance of the insurance company insuring the Vehicle for the same event as the Claim;

p) Copy of the expert report certifying the Destruction due to Total Damage, Total Fire, or Wreckage of the Vehicle,

possibly ordered by another insurance company;

q) Copy of the documents issued by the Public Authority certifying the causes and circumstances of the Claim.

For the purposes of assessing the Claim, the Insurer reserves the right to request additional documentation or to conduct

other types of investigations, such as a technical appraisal or direct investigations into the event that caused the

Claim, and to request additional documentation, which the Insured will be required to provide.

The documentation listed above or otherwise requested is necessary to verify the existence and exact

quantification of the damage and the resulting payment of the Compensation; therefore, in the event of failure to fulfill one or more

of these obligations, the Insurer will not be able to proceed with the payment of the Compensation.

MAPFRE RSA policy

Procedure for Requesting Assistance – Obligations in the Event of a Claim

In the event of a Claim covered by the Insurance, before taking any other action, the Insured must immediately contact the Organizational Structure to resolve the situation.

Pursuant to Article 1915 of the Italian Civil Code, failure to comply with this obligation will result in the forfeiture of the right to Assistance benefits.

To contact the Organizational Structure, which operates 24 hours a day, 365 days a year, the Insured must call the following numbers:

As indicated in the information set (in Art. 9 – "WHAT TO DO IN THE EVENT OF A CLAIM"), the Insured must communicate:

  • License plate number, make, and model of the vehicle requiring assistance;
  • Complete information about the Insured requiring assistance;
  • Address of the location of the vehicle or the Insured requiring assistance;
  • Description of the technical problem that caused the vehicle to be immobilized and any information relevant to the assistance;
  • Services requested;
  • Telephone number where the Organizational Structure can contact the Insured during the assistance process.

MAPFRE DBB Policy

Pursuant to Article 1913 of the Italian Civil Code, in the event of a Claim, the Insured must promptly notify the Insurer in writing no later than 3 days after the Claim occurred or the Insured became aware of it, by sending the Claim report and the documentation indicated in the information set (in Article 7 – "Claim Reporting Procedures") via one of the following methods:

The Insured must communicate:

Pursuant to Art. 1914 of the Italian Civil Code, the Insured is required to do everything possible to avoid or minimize the damage; for

 

example, in the event of Vehicle Theft, immediately report the event to the competent authorities.

Failure to fulfill the aforementioned obligation to avoid or minimize the damage or to notify the Insurer in the event of a Claim may

result, pursuant to Art. 1915 of the Italian Civil Code, in the loss or reduction of the right to Compensation.

For the purposes of compensation payment, the Insured must provide the Insurer with the following documentation:

For all Claims:

a) Clear and detailed description of the accident and any information or information useful for assessing and settling the claim;

b) Contact information (telephone and/or mobile number, email address, etc.) of the Insured;

c) Copy of the vehicle's certificate of ownership and registration document, or copy of the single vehicle registration certificate;

d) Copy of the purchase documents and purchase invoice, indicating the vehicle's purchase price and the dealer's business name;

e) Copy of the current motor third-party liability insurance policy, active and not suspended, at the time the insurance coverage commences and in force, even if suspended, at the time of the claim;

In the case of vehicles that have undergone technical modifications/conversions, copy of the type approval certificate;

g) Claim settlement receipt for the Land Vehicle Hull Policy and a copy of the relevant insurance conditions.

In addition to these documents, the Insured must provide the Insurer with the following additional documentation depending on the type of claim:

In the event of Partial Theft:

h) Copy of the Partial Theft report filed with the Public Authority;

In the event of Partial Fire or Partial Damage to the Vehicle,

i) Copy of the report certifying the Partial Damage or Partial Fire, if requested by another insurance company;

j) Copy of the documents issued by the Public Authority certifying the causes and circumstances of the Claim.

For the purposes of assessing the Claim, the Insurer reserves the right to request additional documentation or to arrange

other types of investigations, such as a technical appraisal or direct investigations into the event that caused the

Claim, which the Insured will be required to provide.

The documentation listed above or otherwise requested is necessary for the purposes of verifying the existence and exact quantification of the damage and the resulting settlement of the Indemnity; therefore, in the event of non-fulfilment of one or more of these obligations, the Insurer will not be able to proceed with the payment of the Compensation.
